ONLINE APPLICATION FOR NEW CONNECTION AND OTHER SERVICES
How to apply for new connection:
- Fill up the Inspection and Assessment Form online through this link.
- Prepare one (1) proof of identity/ authority, such as:
a. Photocopy of any valid ID with picture of the applicant or his/her authorized representative with three specimen signatures
b. Photocopy of any valid ID with picture of the lot owner or his/ her authorized representative with the applicant’s three specimen signatures
c. Notarized Special Power of Attorney (SPA), in case of absence of the principal owner
d. Certificate of birth, death, marriage, extra-judicial settlement, last will & testament, deed of assignment, acknowledgement receipt and secretaries’ certificate to determine authority of signatories - Prepare one (1) photocopy of proof of ownership, such as:
a. Original Certificate of Title (OCT)
b. Transfer Certificate of Title (TCT)
c. Certificate of Award for Urban Poor Development Program, Project Relocation Site
d. Certificate of Land Ownership Award (CLOA) under the Comprehensive Agrarian Reform Program (CARP)
If the abovementioned documents are not available, prepare one (1) photocopy of any of the following documents instead:
a. Tax Declaration (TD), in the name of the applicant – current year
b. Deed of Sale with OCT, TCT, or TD in the name of the previous owner
c. Contract to Sell with OCT, TCT, or TD in the name of the previous owner
d. Deed of Donation with OCT, TCT, or TD in the name of the donor
In the absence of proof of ownership, prepare one (1) photocopy of any of the following documents:
a. Affidavit of Undertaking (notarized)
b. Barangay Certification for Government-owned Lot (signed and sealed)
c. Foreclosed Property Certification from Bank, SSS, GSIS, Pag-IBIG, and others
d. Certification for LGU-assisted Applicants
e. Waiver - One copy of a recent 1×1 photo with white background
How to request for tapping transfer:
- Fill up the Inspection and Assessment Form online through this link.
- Prepare:
a. one (1) photocopy of proof of ownership (if the account owner will transfer to another location)
b. one (1) photocopy of any valid ID with picture of the applicant or his/her authorized representative with the applicant’s three specimen signatures
Note: There are other requirements depending on LCWD’s assessment upon inspection, to wit: (a) Original copy of Barangay Excavation Permit and (b) Three (3) copies of waiver if the application is beyond the 30 meters standard distance from the tapping point.
How to request for change of size:
- Fill up the Inspection and Assessment Form online through this link.
Note: The applicant may be required to bring an original copy of a Barangay Excavation Permit depending on LCWD’s assessment upon inspection.
Important note: When applying online, we request that you submit the electronic copy of the required documents and patiently await communication from our customer service representative. On the other hand, applicants are required to submit the physical copy of the aforementioned requirements when submitting an application onsite.
